Care Provider Registration Requirements

Nurses (RNs and LPNs)

  • Must be 18 years of age or older
  • Current FL RN or LPN license
  • Employment Authorization Document(s) (EAD) to work in the U.S.
  • Current Driver’s License or Photo ID
  • Social Security Card
  • Level 2 Criminal Background Screen
  • Free of Communicable disease statement (dated within the last 6 months. May not be signed by a Chiropractor.)
  • Current CPR from American Heart Association, American Red Cross, or American Safety & Health Institute
  • 1 hour of HIV/AIDS Training
  • 3 hours of Alzheimer's Training
  • 30-hour Infusion Therapy Training (LPN only – Optional)
  • 5-year employment/ contract history
  • Current Auto Insurance – if providing transportation in own vehicle
  • Current Vehicle registration – if providing transportation in own vehicle
  • Employee Identification Number (EIN) (optional)
  • Professional Liability Insurance (recommended)
  • Occupational Accident Insurance (recommended)

Home Health Aides and Certified Nursing Assistants

  • Must be 18 years of age or older
  • Current FL CNA license or proof of a minimum of 40 hours of Home Health Aide Training from an FDoE or DOH approved school or program
  • Employment Authorization Document(s) (EAD) to work in the U.S.
  • Current Driver’s License or Photo ID
  • Social Security Card
  • Level 2 Criminal Background Screen
  • Free of Communicable disease statement (dated within the last 6 months. May not be signed by a Chiropractor.)
  • Current CPR from American Heart Association, American Red Cross, or American Safety & Health Institute
  • 1 hour of HIV/AIDS Training
  • 3 hours of Alzheimer's Training
  • 2 hours of Self-Administration of Medication Training
  • 5-year employment/ contract history
  • Current Auto Insurance – if providing transportation in own vehicle
  • Current Vehicle registration – if providing transportation in own vehicle
  • Employee Identification Number (EIN) (optional)
  • Professional Liability Insurance (recommended)
  • Occupational Accident Insurance (recommended)

Homemakers, Companions, and Sitters

  • Must be 18 years of age or older
  • Employment Authorization Document(s) (EAD) to work in the U.S.
  • Current Driver’s License or Photo ID
  • Social Security Card
  • Level 2 Criminal Background Screen
  • Free of Communicable disease statement (dated within the last 6 months. May not be signed by a Chiropractor.)
  • 1 hour of HIV/AIDS Training
  • 3 hours of Alzheimer's Training
  • 5-year employment/ contract history
  • Current Auto Insurance – if providing transportation in own vehicle
  • Current Vehicle registration – if providing transportation in own vehicle

Case opportunities include days, evenings, nights and extended care shifts.
